• Classes are on Wednesday evenings from 6pm-8pm and the first class is on September 10th, 2025
• At least one parent must attend parent formation nights on Wednesdays from 6:00pm-8:00pm. These classes are offered both in English and Spanish.
• Primary Contact must make sure to be attentively reading/listening to our emails, text messages, calls, and voicemails (If you wish to have two people receiving these communications, please let the Faith Formation Office know)
• Families must attend IN PERSON Sunday (or Saturday Vigil) Mass each weekend at the parish of their choosing. (Online/YouTube Mass is not a valid option)
• Students must complete and turn in their Mass Homework every week to their catechists.
• Parents must ensure that children attend class on a regular basis. (if the child misses more than 3 classes, they may be at risk for not being properly prepared and will be asked to complete another year of preparation)
• Families must be inside the church at 6:00 pm for prayer on Wednesdays (we also do give important announcements during this time so please be on time)
• All students must know all the required prayers in their Student Folder (All tests will be available in English and Spanish, to accommodate the students needs).
• All students must attend mandatory events such as retreats, in-person talks, etc. (a calendar will be provided for you with these dates)
• Families must turn in required documentation no later than September 19th, 2025.
• Our current program for in person students is in English ONLY. We do not offer a program in Spanish. 
• All students must complete their online lessons. (See the Online program - complementary for ALL students' section) 

• For a child to receive the Sacraments of First Holy Communion and Confirmation they must be at least in 3rd Grade, if you have a baptized 2nd grader they can sign up for classes but would only receive their Sacrament of First Reconciliation/Confession. But if you have a 2nd grader who is NOT baptized, they will be prepared for all three sacraments of initiation, the Church views anyone older than 7 as an adult, sacramentally speaking, because they are in the age group where they are most conscious of what they are doing. 
• Baptized 2nd graders will be prepared to receive their sacrament of First Confession. If you have a 2nd grader who is NOT BAPTIZED, they will be prepared to receive all sacraments of Initiation (Baptism, First Communion, and Confirmation) at the same ceremony on Easter.

• ALL registered students must complete our online program (2^nd-12^th graders)
• The online program will not be through a video call set at a specific time rather it will be an online version of what the students are learning in class (Readings, games and assessments are all part of the online program)
• 1-2 Lessons should be completed A WEEK along with passing assessments (with an 80% or higher) given after each lesson, the time and day these lessons are taken DO NOT matter.
• Failure to complete and pass online program can result in child being asked to complete another year of Faith Formation this can be due to the child not yet being properly prepared to receive their sacraments.
• Remember, this is complementary to their weekly classes, and it is intended to help their understanding of our faith.

• Must be at least sixteen years old of age
• Must have received all sacraments of Initiation in the Catholic Church (Baptism, Confirmation, and the Eucharist) 
• Be in good standing with the Church
• Display a visible commitment to living out the Catholic faith and have the ability and intention to carry out his/her responsibility as a spiritual mentor in the candidate’s life
• Does not need to be the same sex as the candidate; however, they CANNOT be a parent of the candidate.
• Godparent/Sponsor may be single but if they are married, they must be married in the Catholic Church.
• Canon Law also states that a sponsor/godparent must not be bound by any canonical penalty. (Cannot be married outside of the Catholic Church)
• The sponsor/godparent should be a Catholic in good standing with whom you will have an ongoing relationship.

• Copy of student’s Birth Certificate
• Copy of student’s Baptism Certificate (if child is baptized. if you misplaced the Baptism certificate you will need to call the church where your child was Baptized to get a copy of the certificate, if your child was Baptized here at SS. Simon & Jude Cathedral, we still need a PHYSICAL copy of the Baptismal Certificate). Please contact our Parish Office to get a copy, they will be happy to help you.
• Completed sponsor form
• Completed registration form for your Child (click below on “Registration”)

1. The Registration fee per children is $130. The full amount of tuition is due at the end of the online registration. . If you wish to pay with cash/check or need asistance, we ask that you please make an appointment with Alexa, and she will assist you with the registration form and take your full payment.
2. The link below will take you to our “Parent Form” This form will be filled out ONCE per family.
3.  After you have filled out the “Parent Form,” on the confirmation page you will receive a message that will display a link, which will take you to the “Child Form” If you are registering multiple children, please note, each individual child will be considered a “registrant” after completing the first child’s form you will see a window pop up in blue you should see a prompt stating ”save registrant” at the bottom right side. This will occur after each additional child. Once you click “save registrant” it will take you to the next page and you will see a prompt in blue that says, “add registrant.” Click on this button and it will set up a new form for you to register your next child. Continue these steps for each additional child. Please make sure to click on “Save registrant” after each form is completed, to ensure each additional child’s form is registered properly.
